Abdominal ultrasound is the first-line and pivotal investigation for hepatic cystic echinococcosis (hydatid disease) caused by Echinococcus granulosus, a zoonosis endemic in pastoralist and sheep-rearing communities. It characterises cyst stage, guides the WHO classification that determines treatment (surgery, PAIR, chemotherapy or watch-and-wait), and detects complications. Because the liver is by far the commonest site, a systematic hepatic survey with attention to internal architecture is essential, complemented by chest imaging for pulmonary involvement.
Indications
- Right upper quadrant pain, hepatomegaly or a palpable mass in a patient from an endemic area
- Incidental cystic liver lesion requiring characterisation
- Known or suspected echinococcal exposure (contact with dogs and livestock)
- Positive echinococcal serology needing anatomical correlation
- Follow-up of cysts treated medically, with PAIR, or postoperatively
- Evaluation of suspected cyst complications (rupture, biliary communication, infection, anaphylaxis)
- Screening of at-risk community members in endemic control programmes
Contraindications & Cautions
- None - ultrasound is safe, non-ionising and repeatable, suitable in pregnancy and children
- Avoid percutaneous diagnostic aspiration of an undiagnosed cyst without cover, given the risk of anaphylaxis and secondary seeding - characterise first with imaging and serology
- Operator dependence and bowel gas may limit assessment; deep or subdiaphragmatic cysts can be difficult
- PAIR (puncture-aspiration-injection-reaspiration), when used therapeutically, is contraindicated for cysts communicating with the biliary tree or that are inactive/calcified
- Interpret with caution in obese patients or where the liver dome is obscured by lung
Patient Preparation
- Fast for 6-8 hours to reduce bowel gas and distend the gallbladder, improving upper abdominal windows
- Explain the examination and obtain verbal consent
- Position supine, with left lateral decubitus and subcostal/intercostal approaches to reach the liver dome
- Use deep inspiration and breath-hold to bring the liver below the costal margin
- Prepare to perform a complete abdominal survey, not just the liver
- Arrange a chest radiograph as an adjunct given the ~20% incidence of pulmonary involvement
Technique & Parameters
- Curvilinear low-frequency probe (2-5 MHz) for the abdominal survey; higher frequency for superficial cysts
- Perform a full survey: liver (right lobe most common), spleen, kidneys, peritoneal cavity and pelvis
- For each cyst record number, size, location (segment), wall thickness/calcification and internal architecture
- Look specifically for daughter cysts, detached membranes (water-lily sign) and hydatid sand/echogenic debris that shifts with position
- Apply the WHO-IWGE classification (CL, CE1-CE5) to stage activity
- Use colour Doppler to confirm avascular cyst content and distinguish from vascular lesions
- Adjunctive chest radiograph for pulmonary cysts; CT or MRI/MRCP where available to assess biliary communication, extrahepatic and complicated disease
- Correlate with echinococcal serology (ELISA/immunoblot) to support the diagnosis
Systematic Review
- Survey the entire liver segment by segment, then spleen, kidneys, peritoneum and pelvis for additional cysts
- Characterise the cyst wall: single vs double-layered ('double-line' sign), thickness and calcification
- Assess internal content: purely anechoic, snowstorm/hydatid sand, daughter cysts (cysts-within-cyst), or solid/heterogeneous matrix
- Detect detached endocyst membranes floating within fluid (water-lily/serpent sign)
- Assign the WHO stage (CL, CE1-CE5) to guide management
- Evaluate for mass effect on adjacent biliary tree and vessels and for biliary dilatation
- Search for complications: rupture into peritoneum/biliary tree, infection (heterogeneous echogenic content, gas), and communication
- Check the chest (via radiograph) and note any other systemic sites
Key Findings & Significance
- CL (cystic lesion): a unilocular anechoic cyst without a visible wall - undifferentiated and requires correlation
- CE1 (active): unilocular anechoic cyst with a visible double-line wall and often hydatid sand producing a 'snowstorm' on shaking
- CE2 (active): multivesicular/multiseptate cyst with daughter cysts in a honeycomb or rosette/wheel-spoke pattern - pathognomonic
- CE3 (transitional): CE3a with detached membranes producing the water-lily sign, or CE3b with daughter cysts in a solid matrix
- CE4 (inactive): heterogeneous hypoechoic/hyperechoic degenerated content ('ball of wool') without daughter cysts
- CE5 (inactive): thick calcified wall with partial or complete acoustic shadowing
- Water-lily sign (floating collapsed membranes) and daughter cysts are the most specific diagnostic features
- Complications: cystobiliary communication with biliary dilatation, cyst infection, and rupture with peritoneal seeding raising anaphylaxis risk
Differential Considerations
- Unilocular anechoic cyst (CL/CE1) -> simple hepatic cyst vs biliary cystadenoma vs early hydatid - use double-line sign and serology
- Multiseptate cyst with daughter cysts (CE2/CE3b) -> hydatid (characteristic) vs biliary cystadenoma/cystadenocarcinoma vs mesenchymal hamartoma
- Membranes within a cyst (CE3a) -> hydatid water-lily vs haemorrhagic cyst with septa vs abscess
- Heterogeneous solid-appearing lesion (CE4) -> degenerated hydatid vs pyogenic/amoebic abscess vs necrotic tumour
- Calcified lesion (CE5) -> inactive hydatid vs calcified granuloma/old abscess vs calcified metastasis
- Right upper quadrant cystic mass with fever -> complicated/infected hydatid vs amoebic liver abscess vs pyogenic abscess (endemic overlap)
Pearls & Pitfalls
- Daughter cysts and the water-lily sign are the most specific sonographic signs - actively look for them before calling a lesion a simple cyst
- Shaking the patient can mobilise hydatid sand to produce a 'snowstorm', supporting an active cyst
- Do not aspirate an uncharacterised cyst diagnostically - spillage risks anaphylaxis and peritoneal dissemination
- The double-line (double-layered wall) sign helps distinguish CE1 from a simple hepatic cyst
- An apparently simple cyst in an endemic patient should prompt serology and possible follow-up rather than dismissal
- CE4 'ball-of-wool' lesions can mimic solid tumours - correlate with serology and prior imaging
- Assess for biliary communication before considering PAIR; cystobiliary fistula contraindicates scolicidal injection
- Combine the WHO stage with serology to plan management: active CE1/CE2 often need intervention, inactive CE4/CE5 may be observed
Structured Report
- State fasting status, technique and completeness of the abdominal survey
- Report number, size, hepatic segment and location of each cyst
- Describe wall (thickness, double-line, calcification) and internal architecture (daughter cysts, membranes, matrix)
- Assign the WHO-IWGE stage (CL, CE1-CE5) for each cyst and state active vs inactive
- Report complications: biliary communication/dilatation, infection, rupture and mass effect
- Note extrahepatic sites and recommend chest imaging for pulmonary disease
- Impression: likelihood of hydatid disease, activity stage and management-relevant complications
- Recommend serology correlation and appropriate pathway (albendazole, PAIR, surgery or watch-and-wait) and referral
