Categories & management

| 0 | Incomplete | — | Additional lung cancer screening images and/or comparison with priors. |
| 1 | Negative | <1% | Continue annual screening (LDCT) in 12 months. |
| 2 | Benign appearance/behavior | <1% | Continue annual screening in 12 months. |
| 3 | Probably benign | 1–2% | 6-month LDCT. |
| 4A | Suspicious | 5–15% | 3-month LDCT; PET/CT may be used when there is a ≥8 mm solid component. |
| 4B | Very suspicious | >15% | Chest CT ± contrast, PET/CT, and/or tissue sampling per multidisciplinary review. |
| 4X | Category 3/4 with additional suspicious features | >15% | Manage as 4B — further workup / tissue sampling. |
